At Partner for Mental Health we operate from a person-centered and trauma-informed position that understands that service systems can be intimidating, complex, uncoordinated, and possibly inadvertently re-traumatizing. We know that the most common symptoms of mental illness include disorganized thinking, racing thoughts, poor concentration and short-term memory, and feelings of hopelessness and helplessness. We recognize the social isolation that mental illness can cause.
The combination of complicated service systems, lack of social support, and illnesses that may prevent clear thinking and limit motivation, work together to make it hard for individuals to get what they need.
Identifying services, finding available providers, making and keeping appointments, and addressing the external factors that impact mental health—physical health needs, housing, employment, opportunities for community inclusion, and education—can require a level of executive functioning that mental illness itself prevents people from accessing. This is where our Systems Navigators can help.
Our Systems Navigators are unbiased and independent brokers of supports and services. We understand how scary and difficult it can be to figure out where to go to get what you need, whether what you need is trauma counseling or stable housing. Partner for Mental Health does not provide clinical or human services ourselves; instead we help people successfully connect to the existing local resources best positioned to help them accomplish their goals.
Partner for Mental Health has learned that most people with mental illness know what it is they need to live stable lives in the community. Our Systems Navigators remove the barriers that stand in their way of attaining what they need and help facilitate meaningful recovery, resulting in:
· Improved physical and mental health outcomes
· Increased housing and financial stability
· Increased Social support and community inclusion
· Reduced incarceration rates
· Reduced unnecessary hospital utilization
· Reduced rates of substance abuse
Systems Navigation is person-centered, trauma informed, and evidence based. We are set apart by our flexibility, comprehensiveness, intensity, and commitment to building partnerships to help people meet their personal recovery goals.
